Job summary

The Phoenix Rehabilitation and Nursing Center is seeking a reliable Home Health Aide (HHA) to assist with safe resident transport inside and outside the facility. This role focuses on mobility support and compassionate care for seniors, ensuring timely arrival to appointments and activities while maintaining safety and comfort.

Responsibilities include transporting residents, assisting with transfers, communicating with the care team, and following all safety guidelines.

Qualifications

• HHA certification in good standing.
• Valid driver's license and clean driving record (if transporting offsite).
• Ability to lift and assist residents safely.
• Strong communication and interpersonal skills.
• Compassionate, patient, and resident-focused.
• Prior experience in a skilled nursing or rehabilitation setting preferred but not required.

Responsibilities

• Safely transport residents to and from medical appointments, therapy sessions, and facility activities.
• Provide assistance and support to residents during transfers and mobility challenges.
• Monitor and ensure resident safety and comfort during transport.
• Communicate effectively with the care team regarding resident needs and observations.
• Assist with light resident care and companionship as needed.
• Follow all facility policies, procedures, and safety guidelines.
