HumanGood is seeking a Social Services Coordinator who will organize, implement, and evaluate the Social Services program to meet residents' psycho-social needs. The role includes resident assessment, records maintenance, and ensuring residents connect with needed supportive services to live independently in the community.

The position is full-time in Trevose, PA, with a typical schedule Monday–Friday, 9:00 AM–4:00 PM. Benefits and a supportive team environment are highlighted in the job posting.

Qualifikationen

• 1+ years of case management experience required.
• 2+ years of social services experience, preferably in a senior care setting.
• Bachelor's Degree in social work or related program required (gerontology, psychology, or recreation therapy).
• Social Services certification preferred.

Our Social Services Coordinators are passionate, organized, and compassionate towards residents. You would be responsible for organizing, implements and evaluates the Social Services program to meet the psycho-social needs of residents. This may include resident assessment, maintenance of resident records, assuring that residents are linked to the specific supportive services they need to continue living independently in the community.

Schedule: FULL-TIME

• Monday - Friday at 9:00 AM - 4:00 PM

Pay Range: $23.00 - 25.00/hour, depending on experience

To Be Successful In The Role, You Would Have

• 1+ years of experience in case management required.
• 2+ years of social services experience, preferably in a senior care setting.
• A Bachelor's Degree required in social work or related program such as gerontology, psychology, or recreation therapy.
• Social Services certification preferred
