The role involves discharge planning, care plan development, resident assessments, and advocating for residents’ rights and dignity, while collaborating with administration, nursing, and medical staff to ensure compassionate, person-centered care.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social services programs.
• Meet with administration, medical and nursing staff to coordinate social services.
• Participate in discharge planning and care plan development and resident assessments.
• Encourage resident and family involvement in care planning and attend care plan meetings.
• Assist residents in obtaining transportation to medical appointments.
• Document social service actions accurately in each resident’s medical record.
• Support policy development and annual review to improve quality of care.
• Educate staff on Resident’s Rights and dignity of residents.
• Advance Care Planning and review of Advance Directives with residents or representatives.
• Advocate for residents and report any suspected abuse or neglect to appropriate authorities.
• Facilitate access to treatment and resources for residents with psychosocial needs.
