Role Description

The Resident Care Director is a full-time, on-site role based in San Jose, CA, responsible for overseeing the delivery of high-quality care and services to residents. This role coordinates individual care plans, supervises care team members, and ensures compliance with clinical standards, regulatory requirements, and community policies. Daily responsibilities include assessing residents' health and wellness, managing case reviews, collaborating with physicians and families, and maintaining accurate documentation. The Resident Care Director also leads training for care staff, supports emergency and critical situations, and works closely with community leadership to enhance resident experience and safety.

Qualifications

• Clinical care skills, including Nursing and Medicine, with the ability to oversee and support resident health needs.
• Experience in Case Management, including care planning, coordination of services, and resident advocacy.
• Background in Critical Care Medicine or managing acute or complex clinical situations in a senior living or similar setting.
• Training skills to coach, mentor, and educate care team members on best practices and regulatory standards.
• Valid and active nursing license (RN or LVN/LPN) in the state of California, or equivalent applicable credential.
• Previous leadership experience in senior living, long-term care, assisted living, or related healthcare environment.
• Strong communication, interpersonal, and conflict resolution skills, with a resident-centered and family-focused approach.
• Ability to work on-site, manage multiple priorities, and maintain accurate and timely documentation in electronic health records.
• Knowledge of state and federal regulations related to senior housing and resident care; dementia care experience is a plus.
