Qualifications

• Must be at least eighteen (18) years old.
• Must be able to lift 25+ pounds.
• Ability to handle physical aspects of a job including bending, stooping, lifting and walking.
• Demonstrate the ability to communicate adequately to complete forms and follow instructions.

Responsibilities

• Completes 90-day orientation.
• Provide personal care tasks including hygiene, feeding, ambulation, and health monitoring.
• Ensure safety and maintain a safe environment for individuals served.
• Assist with toileting and self-administration of medications (with training).
• Plan meals, shop for groceries, prepare/serve food, and clean up after meals.
• Accompany individuals to appointments and run errands.
• Maintain ISP System documentation and monthly checklists.
• Assist with staffing needs and participate in on-call rotation.
• Follow all policies, procedures, and regulations and assist with annual trainings.
