Job summary

MorningStar Senior Living is seeking a Resident Care Coordinator to lead care operations in our senior living community. You will supervise Care Managers and MCMs, monitor resident needs, liaise with families, and coordinate services with Wellness, Dietary, Maintenance and Housekeeping teams.

The role requires overseeing care plans, ensuring compliance with state and federal regulations, and fostering a compassionate, service-oriented culture for residents and their families.

Qualifications

• Two years of health-related supervisory experience preferred in long-term care or assisted living.
• Experience with memory-impaired senior population.
• State-approved Medication Administration training and first aid certificate required.

Responsibilities

• Provide maximum resident-care services within limits defined by delegated tasks.
• Plan, organize and direct the operation of the Reflections and Assisted Living Neighborhood in compliance with regulations and MorningStar policies.
• Design, schedule and facilitate the Memory Care Program with dementia-appropriate activities.
• Coordinate Negotiated Service Plans to ensure tasks including activities of daily living are completed.
• Support Move-In assessments and communicate resident needs to the team.
• Create and monitor Daily Assignment Sheets and adjust staffing as needed.
• Communicate with families through meetings, emails and calls.

Summary of Role:

The Resident Care Coordinator must exhibit genuine interest in residents and show patience and understanding while working with residents, families, and visitors. This position is responsible for managing all facets of service to address resident care needs in the community. The RCC hires, trains, supervises, and schedules the Care Managers and MCMs. The RCC monitors resident care needs, alerts the Wellness Director when the resident has a change in condition, addresses resident and family concerns, and acts as a liaison with community providers. The RCC coordinates services with the Life Enrichment, Dietary, Maintenance and Housekeeping departments for the Reflections and Assisted Living areas. The person holding this position is delegated the responsibility for carrying out the assigned duties and responsibilities in accordance with current existing federal and state regulations and established company policies and procedures.

Duties and Responsibilities of the Position:

• Provide maximum resident-care services within limits defined by delegated tasks.
• Plan, organize, develop and direct the overall operation of the Reflections and Assisted Living Neighborhood in accordance with federal, state, local laws and MorningStar Senior Living regulations and guidelines.
• Design, schedule and facilitate the Memory Care Program ensuring a variety of dementia appropriate activities.
• Facilitate and monitor the Negotiated Service Plans to ensure completion of tasks including activities of daily living and engagement in daily programming.
• Participate in Move-In process by attending initial assessment with Wellness Director and communicating resident needs and preferences to team members.
• Create and monitor the Daily Assignment Sheets.
• Utilize the assessment process and findings to determine the care levels and appropriate variable staffing.
• Communicate regularly with families through family council meetings, Negotiated Service plan meetings, emails, and phones calls.
• Act as a resource and meet the family’s social and emotional needs while encouraging family involvement.
• Schedule all Resident Care Managers, Lead Care Managers, and partners with Wellness Director in scheduling Medication Care Managers.
• License/Certification: Successful completion of State approved Medication Administration training and a current first aid certificate as required by the State.
• Education/Experience: Two years of health-related supervisory experience preferred in long-term care or assisted living and experience with memory impaired senior population.
