Responsibilities

  • Assist the Director of Health & Wellness with administrative and operational duties within the Wellness Department.
  • Coordinate resident care needs including scheduling, documentation, and required clinical logs.
  • Develop and maintain resident care staffing schedules with a four‑week rolling cycle.
  • Track team member call‑offs and assist with scheduling coverage.
  • Maintain resident care scheduling records and approved time‑off documentation.
  • Perform daily filing and organization of resident clinical records and ensure physician signatures when required.
  • Retrieve and track signed physician orders within required timeframes.
  • Assist with medication cart audits and medication administration reviews.
  • Monitor medication passes for accuracy and timeliness; provide coaching as needed.
  • Assist with MAR reviews and monthly reconciliations.
  • Track and manage lab orders and documentation for scheduled draw days.
  • Prepare documentation for in‑house physician visits and coordinate resident medical appointments.
  • Conduct Memory Care room safety checks as assigned.
  • Order wellness supplies and maintain inventory.
  • Support new hire orientation and training for Care Managers and Medication Technicians.
  • Assist with evaluations of Care Managers and Medication Technicians.
  • Perform building rounds to assess resident care delivery and address needs.
  • Communicate changes in resident condition to the Director of Health & Wellness or physician with proper documentation.
  • Participate in Manager on Duty rotation and support safety procedures.
  • Perform other duties as assigned.

Resident Care Coordinator

As a Resident Care Coordinator, you’ll play an essential role in enriching the lives of seniors, creating meaningful connections, and ensuring resident care services are delivered safely, accurately, and in compliance with company policies and state regulations.

  • Schedule: FLEXIBLE SHIFT, EVERY OTHER WEEKEND AVAILABILITY
  • Location: 122-124 GREEN AVENUE, WOODBURY, NJ 08096
  • Rate of Pay: $22.00 TO $23.00 AN HOUR
What You’ll Do
  • Assist the Director of Health & Wellness with administrative and operational duties within the Wellness Department.

  • Assist with ordering wellness supplies and maintaining inventory.
  • Support new hire orientation and training for Care Managers and Medication Technicians.
  • Assist with evaluations of Care Managers and Medication Technicians.
  • Perform building rounds to assess resident care delivery and address needs.
  • Ensure resident rights are upheld at all times.
  • Communicate changes in resident condition to the Director of Health & Wellness or physician when required, ensuring proper documentation.
  • Participate in Manager on Duty rotation as assigned.
  • Support fire safety, disaster preparedness, and emergency procedures.
  • Perform other duties as assigned.
Qualifications
  • Certified Nurse’s Aide (CNA) certification and/or Qualified Medication Aide (QMA) certification.
  • High School diploma or GED.
  • Minimum age of 18.
  • CPR certification.
  • First Aid certification.
  • Previous experience working with seniors preferred.
  • Ability to communicate effectively with residents, families, and team members.
  • Ability to read, interpret, and complete required documentation and logs.
  • Strong organizational, time management, and multitasking skills.
  • Ability to remain calm and make sound decisions in urgent or stressful situations.
  • Demonstrated good judgment, problem‑solving, and decision‑making skills.
  • Ability to work indoors in a clinic.
