Job summary

Cogir Senior Living in the United States is seeking a Resident Care Coordinator to act as a liaison among residents, families, and the care team, helping schedule, supervise staff, and ensure quality assisted living services. You will assist with medication administration and provide direct resident care when necessary.

The position requires a state medication administration license (CNA/HCA) and a minimum of two years’ care experience, with supervisory experience preferred.

Qualifications

• High School diploma or equivalent is required.
• Must hold a valid state medication administration license (CNA/HCA) and meet state rules for assisted living.
• Certificates in Nurse Delegation or Diabetes Specialty are desirable or willing to obtain.
• An LPN license is a plus; First Aid/CPR certification is required or obtainable.

Responsibilities

• Schedule and assign caregiving and medication technician shifts to ensure coverage and compliance.
• Supervise, train, and support care team members to meet care standards.
• Monitor the medication room and orders in collaboration with staff.
• Maintain accurate resident records including care plans and reports.
• Coordinate care planning with home health agencies and outside providers.
• Assist with admissions and coordinate with sales team for move-ins.
• Facilitate communication among residents, families and healthcare providers.
• Support medication administration and provide direct care when needed.
• Understand state care regulations and implement them locally.

POSITION SUMMARY

Our Resident Care Coordinator serves as a liaison between residents, their families, and the care team, promoting a supportive and safe living environment. They support the Health and Wellness Director by assisting in scheduling, supervising staff, and ensuring that systems are in place to provide quality assisted living services in all resident care areas. Duties include problem-solving resident concerns and coordinating care with the team and external providers. Additionally, they help with medication administration and direct resident care when necessary.

KEY RESPONSIBILITIES

• Schedule and assign caregiving and medication technician shifts to ensure adequate coverage and compliance with staffing guidelines and department allowances.
• Participate in the recruitment and interview process for new care team members.
• Supervise, train, and support care team members, promoting teamwork and adherence to care standards.
• Supervise the medication room and orders, collaborating with medication technicians and caregivers.
• Maintain accurate and up-to-date resident records, including care plans, incident reports, and medical documentation.
• Coordinate care planning with home health agencies on-site, working within community and company policies.
• Coordinate admissions with the sales team, including supervising move-ins.
• Complete family/resident admission interviews.
• Facilitate effective communication among residents, their families, and healthcare providers to ensure seamless care delivery.
• Address residents’ special needs, requests, and complaints.
• Assist with medication administration and provide direct care to residents as needed.
• Understand the community's care regulations, as well as state-specific regulations.

Requirements

CANDIDATE QUALIFICATIONS

Education and Certifications:

• High School Diploma or equivalent.
• Must have a valid state medication administration license (such as CNA/HCA) and meet the state requirements for medication administration in assisted living.
• Nurse Delegation and Diabetes Specialty Certificates, or willingness to obtain within a prescribed period.
• An LPN nursing license is a plus.
• Current First Aid and CPR license or ability to obtain.

Experience, Competencies, and Skills:

• At least two (2) years of experience administering medications and providing care in a professional caregiving setting is required.
• At least one (1) year of relevant supervisory experience, including staff scheduling, is preferred.
• Excellent attention to detail and high integrity.
• Excellent communication skills and teamwork mindset.
• Strong leadership and team management skills.
• A positive attitude, empathy, patience, and a commitment to treating our residents with dignity and respect.
• Willingness to participate in weekend rotation.
