Job summary

Nexus Health is seeking a social services leader for our skilled nursing facility to identify residents’ psychosocial and emotional needs and facilitate access to services in line with ethical and regulatory standards.

You will develop and oversee social health programs, ensure thorough documentation in medical records, and lead a team to uphold residents’ rights and high-quality care. Strong communication, teamwork, and professional development are essential in this role.

Qualifications

• Bachelor’s degree in human service-related field required.
• Strong verbal and written communication skills expected.

• Participation in professional organizations and workshops.

Responsibilities

• Development and implementation of all social health programs in the setting.
• Oversight of documentation of social work services in residents' medical records; adherence to standards.
• Maintain legal, ethical, and professional standards in records and reporting.
• Establish and manage a social work program aligned with professional standards and laws.
• Establish and oversee quality assurance procedures and adjust as needed.
• Ensure staff understand residents’ rights per applicable laws.
