Job summary

EverTrue is seeking a dedicated Personal Care Aide to provide supportive and functional care to residents, ensuring they remain the director of their own care. You will work with an interdisciplinary team to carry out daily living activities and maintain a safe, respectful environment.

Responsibilities include assisting with personal care, following EverTrue policies, documenting observations, and supporting residents during meals and activities.

Qualifications

  • Previous resident/patient care experience preferred.
  • If working in Pennsylvania, a High School diploma, GED, or active registry status on the PA state nurse aide registry is required.
  • Ability to read and comprehend simple instructions, short correspondence, and memos; Ability to write simple correspondence.
  • Ability to effectively present information in one on one and small group situations.
  • Basic computer skills, limited to retrieving data and data entry.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must have patience and be willing and able to demonstrate professional behavior in interactions with difficult situations or individuals, including residents and resident families.

Responsibilities

  • Supports residents in attaining highest practicable level of functioning with personal care and activities of daily living.
  • Provides care per current standards and EverTrue policies.
  • Maintains resident care environment; responds to requests promptly.
  • Follows safety procedures and reports hazards or incidents to nursing leadership.
  • Performs resident observations and documentation; participates in care planning.
  • May escort residents to dining area and monitor mealtimes.
  • May document vital signs (BP, temp, respiration, weight).
  • In PA, may serve as administrator designee in absence of PC/AL director; escalate as needed.

Summary

Provides supportive and functional care to residents with routine daily care in accordance with our established community care procedures and as may be directed by nursing leadership; Works collaboratively as a member of the interdisciplinary team with a focus on maintaining the resident as the director of their own care.

Job Responsibilities
  1. Supports residents in attaining and/or maintaining their highest practicable level of functioning by providing and/or assisting residents with personal care and performance of all routine activities of daily living

  2. Provides care and services in accordance to current standards of practice as defined by training standards and EverTrue Clinical Policies and Procedures

  3. Maintains the appropriate resident care environment and responds to resident requests for assistance in a timely manner

  4. Follows all safety procedures and protocol as dictated by EverTrue policy, federal and state regulations and standard nursing practice and reports all hazardous conditions, defective equipment and accidents or incidents to their nursing leader immediately

  5. Performs and provides proper resident observations and record keeping; Functions as the resident’s advocate, participates in care planning and communicates to their nursing leader and interdisciplinary team any changes observed

  6. May be instructed to assist and escort residents to/from dining room and in-house activities; monitor residents in the dining room at mealtimes

  7. May be assigned the responsibility of documenting residents’ vital signs including blood pressure, temperature respiration, weight checks, and other pertinent information

  8. If working in the state of Pennsylvania, may be assigned to serve as the administrator designee (if aged 21 years or older) in the absence of the “Personal Care Administrator” or PC/AL Clinical director; Evaluates immediate circumstances and contacts highest level of authority to discuss next steps

Is willing to work beyond normal working hours and on weekends and holidays when necessary. Is subject to exposure to fumes or airborne particles, and toxic or caustic chemicals. Is subject to exposure to infectious waste, diseases, conditions, etc., including exposure to the AIDS and Hepatitis B virus within guidelines of standard precautions. Appropriate ergonomic and personal safety equipment is provided as needed.

EverTrue Pay & Benefits

Pay grade: 07, $15.00/hour to $22.00/hour. Eligible for shift differentials: Yes. $1.50/hour evenings; $1.00/hour nights; $1.00/hour weekends (States of IL & MO; May vary in other states)
