Job summary

Fresenius Medical Care is seeking a Registered Dietitian to provide nutrition services for in-center and home patients, collaborating with the interdisciplinary healthcare team to optimize nutritional status and outcomes.

The role involves assessing patient knowledge, delivering education on diet related to kidney disease, and developing individualized care plans. Strong communication, organizational, and computer skills are required, with renal nutrition experience preferred.

Qualifications

• 1 year or more of professional clinical nutrition experience as a Registered Dietitian.
• Renal nutrition experience preferred.
• Demonstrated leadership, organization, and communication skills.

Responsibilities

• Assess patients’ diet knowledge and provide education on diet and kidney disease.
• Develop individualized care plans with the interdisciplinary team.
• Document nutrition assessments and progress notes per FMCNA policies.
• Monitor lab results and adjust nutrition therapy to improve status.
• Coordinate with Case Management for nutrition-related referrals and education.
