Job summary

Riverside Health System in Gloucester, Virginia is requesting a Licensed Practical Nurse (LPN) to provide patient-centered care under supervision of a physician or registered nurse. You will perform focused assessments and document care, supporting plans and evaluating treatment effectiveness.

This role includes lab tasks, vaccine administration, medication processing, and collaboration with a multidisciplinary team to ensure high-quality patient care while maintaining HIPAA and OSHA standards.

Qualifications

• Education Program Graduate and Licensed Practical Nurse credential (LPN) required.
• 1 year nursing experience preferred.
• LPN license required upon hire with Virginia Department of Health Professions.
• CPR/BLS certification required upon hire.

Responsibilities

• Completes clinical documentation accurately per Riverside Medical Group standards.
• Maintains documentation for QA including point-of-care testing, medication logs and wellness visits.
• Collaborates with providers to address care gaps for each patient.
• Ensures documentation is accurate, legible and completed on the same day.
• Performs lab-related tasks and proficiency testing.
• Maintains clinical competency per Riverside Medical Group expectations.
• Updates and verifies patient records by interviewing patients and confirming visit purpose.
• Prepares patients for examinations by obtaining vital signs.
• Prepare and sanitize exam rooms before and after appointments.
• Administers medications and vaccines under physician supervision.
• Assists with processing prescription refills and forwards to pharmacies.
• Maintains medical inventory and stocking of exam rooms.
• Keeps equipment in working order and coordinates preventive maintenance.
• Ensures compliance with HIPAA, OSHA, and scope-of-practice regulations.
• Completes required competencies and education to maintain safe patient care.
• Educates patients about medications and care plans as needed.
• Explains the importance of patient history forms for care.
• Provides timely patient follow-up on lab results, calls, refills, referrals and scheduling.
• Answers calls from patients, pharmacies, physicians, and vendors by close of business.
• Communicates lab/pathology results and schedules additional tests as ordered.
• Supports reimbursement through pre-authorization of meds, procedures, and tests.
• Contributes to a multidisciplinary team to support patient care needs.
• Participates in multi-disciplinary team meetings as assigned.
