Job summary

Interwell Health is hiring a Renal Care Coordinator Telephonic (RCCT) to support nephrology practices and patients from a remote, full-time position. The RCCT will educate, coordinate, and help manage late-stage CKD patients as they transition toward dialysis and related therapies.

You will liaise with staff, explain modality options and access types, and use motivational interviewing to encourage patient engagement and adherence to care plans while supporting optimal health outcomes.

Qualifications

• Minimum 6 months experience as Renal Care Coordinator.
• 1–2 years RCC experience preferred.
• Experience with renal transplant, dialysis or CKD care.

Renal Care Coordinator Telephonic (RCCT) are clinical professionals providing support telephonically to nephrology practices and their patients. As a RCCT, you will work to organize the care of late-stage chronic kidney disease patients by providing support, education, and care coordination services with the goal of those patients having an optimal start to dialysis. Services are generally provided telephonically. Note: This is a full time, remote position.

What You'll Do:

• Assesses patients' knowledge of late-stage CKD and treatments, educating and informing patients to enable them to make informed decisions regarding the steps to manage health issues during the transition to RRT. Provides support, guidance, and coordination of care for patients seeking conservative care or palliative care.
• Acts as a liaison with appropriate staff to ensure every patient and family member (if applicable) receives comprehensive information on specific modality advantages and disadvantages, hemodialysis treatments both at home and in-center, peritoneal dialysis, kidney transplantation and conservative care, as well as education on hemodialysis access types with focus on the health and safety benefits of AV Fistula or AV-Graft compared to central venous catheters.
• Organizes the Nephrology Practice late-stage CKD patient population regarding CKD education, including modality selection, permanent access placement and maintenance and a stable transition to RRT.
• Utilize motivational interviewing techniques to influence members to engage in their own care.
• Participates in the interpretation of summary clinical data and its use in improving late-stage CKD care processes.

What You'll Need:

• Minimum of 6 months previous experience as a Renal Care Coordinator with Interwell Health; 1-2 years of prior RCC experience highly preferred.
• A combination of renal transplant, dialysis, or CKD patient care required.
• Strong organizational and communication skills.
• Must be available to work hours of the time zone of practices supported.
