Responsibilities

• Assess appropriate treatment plans across levels of care using evidence-based standards.
• Coordinate, monitor, and evaluate options to facilitate healthcare services/benefits for members.
• Identify members at risk and facilitate referrals for integrated care and programs.
• Provide triage and crisis support and aid in discharge planning.
