Responsibilities include assessing health status, closing care gaps, coordinating referrals, and ensuring continuity of care with a patient-centric approach.

Qualifications

• Current, unrestricted RN license in the state of residence.
• Compact RN license with ability to obtain licenses in other states.
• 3+ years of RN experience in hospital, acute care or direct care.
• 1+ years with solid organ transplant or VAD patients.
• Proficient in computer use and Windows navigation.
• Reliable high-speed internet at home with dedicated workspace.

Responsibilities

• Make outbound calls to assess members' health status.
• Identify gaps or barriers in treatment plans.
• Provide patient education for self-management.
• Make referrals to outside sources.
• Provide continuum of quality care through close phone communication.
• Assist with condition education, medication reviews and resource connections.
