Job summary

ChenMed is seeking an Advanced Practice Provider I, CareLine to deliver diagnostic care through virtual telemedicine visits and phone consultations. The role includes after-hours and weekend coverage as the dispositional authority, with documentation and patient education as core duties.

The position requires state-specific licensure and board certification, with travel to Georgia or Tennessee as mandated, and a load that includes evening/overnight/weekend shifts.

Qualifications

  • Bachelor’s degree in Nursing and graduate of a school of nursing for Advanced Practice Nursing with certification in area of specialty required; Master’s degree in Nursing required.
  • Board certification by AANP or ANCC required.
  • Basic Life Support (BLS) certification required at hire.

Responsibilities

  • Assess acute and non-acute clinical problems via telemedicine; document health data and develop a management plan.
  • Coordinate patient care with PCPs and other providers; ensure guideline compliance.
  • Use telemedicine to achieve optimal patient outcomes and monitor progress.
  • Collaborate with on-call PCP to support clinical outcomes and policy adherence.
  • Contribute to telehealth quality improvement and program effectiveness measures.
  • Provide professional communication and collaboration with physicians, patients and team members.
