Navitus Health Solutions is expanding and seeks a Telehealth RN Case Manager to deliver MTM services and conduct clinical outreach for commercial and Medicare members. The role focuses on enhancing member health and wellness through tele-pharmacy and coaching within our Clinical Engagement Center.

The ideal candidate is an RN with unrestricted licensure, at least two years of clinical experience, and comfort working in a mult-disciplinary team while staying current with CMS guidelines and HIPAA

Qualifications

• RN with unrestricted licensure.
• Two years clinical experience preferred.
• Experience in a multi-disciplinary team is preferred.
• Knowledge of HIPAA regulations and healthcare industry practices.

Responsibilities

• Conduct MTM and expanded clinical programs per CMS.
• Develop clinical algorithms, pathways, and call scripts.
