CareTalk Health, a fully remote, nationwide virtual medical practice based in the United States, is seeking experienced Nurse Practitioners to deliver comprehensive virtual care. You’ll conduct Annual Wellness Visits and chronic disease management by phone or video, with scheduling, nursing support, and tech help provided so you can focus on patients.

Applicants must hold an active, unrestricted NP license in multiple states and have telehealth or virtual care experience.

Qualifikationen

• Active, unrestricted NP license in multiple states.
• Experience providing care in a virtual telehealth clinic.
• Experience in preventive and/or primary care.
• Comfort conducting phone and video visits; strong documentation.

Aufgaben

• Conduct Annual Wellness Visits and follow-up visits by phone or video.
• Manage chronic conditions and review medications; prescribe where appropriate.
• Write orders for indicated screenings and review results.
• Close care gaps and build rapport with Medicare patients.
• Document encounters in SOAP format.
