Qualifications

• Bachelor's Degree in social work or related field (or equivalent).
• Preferred: Master's in Social Work and 1 year related experience.

Responsibilities

• Coordinate client intake and referrals; assess problems or needs for intervention.
• Develop, write, implement and monitor strength-based service plans.
• Determine resources and referrals to community-based care and services.
• Develop and implement abuse prevention plans; address vulnerabilities.
• Determine and implement intervention services; assess risks to self and others.
• Intervene in crisis situations and coordinate emergency services.
• Negotiate and resolve conflicts; provide client case management.
• Collaborate with professionals and community agencies; maintain case records.
• Prepare required narratives, eligibility reviews and court-related documentation.
• Provide testimony or written recommendations in court or administrative hearings.
• Advise clients on resources, rights and responsibilities; deliver program information.
