Qualifications

• High school diploma or GED required.
• Three years of broad clinical experience.
• Three years in home care, discharge planning, case management, or related healthcare setting.
• Active, unrestricted New Jersey license (RN/LPN/SW/CSW).

Responsibilities

• Assess members' clinical, functional, and psychosocial needs using established guidelines.
• Develop, coordinate, and support implementation of individualized, member-centered care plans.
• Coordinate care with members, families, physicians, hospitals, and community resources.
• Provide field-based and telephonic case management for SNP members.
• Maintain accurate documentation in accordance with standards and policies.
