Job summary

CenterWell Senior Primary Care is seeking a Clinical Pharmacist with over 2 years of experience to join our primary care organization. You will improve patient outcomes through medication management, patient education, and collaboration with our healthcare team.

You will report to a Associate Director, Integrated Clinical Pharmacist. You will use evidence-based medicine to evaluate pharmacotherapy, participate in high-risk rounds, optimize drug coverage, and document activities in EMR while

Qualifications

• Active and unrestricted Pharmacist license.
• Pharm.D. or other advanced pharmacy degree.
• 2+ years' experience as a practicing pharmacist.
• Intermediate level or higher competency with Microsoft Office programs.

Responsibilities

• Use evidence-based medicine and clinical judgment to evaluate pharmacotherapy for high-risk patients.
• Execute clinical protocols per organization and state regulatory agencies.
• Optimize drug coverage benefits and reduce costs.
• Collaborate with physicians, nurses, and other healthcare professionals.
• Participate in high-risk rounds with a multidisciplinary team.
• Provide excellent patient counseling on complex medication information.
• Document all activity in EMR and management systems.
• Support CMS Stars and HEDIS quality measures for clinic performance.
• Conduct in-service education and develop educational materials.
