Job summary

Range Mental Health Center in Minnesota is seeking a School-Linked Behavioral Health Practitioner and Targeted Case Manager to support students' emotional and behavioral well-being in offsite settings such as schools, homes, or community sites.

The role encompasses skills training, targeted case management, crisis intervention, and treatment plan development, with travel between sites as needed. It is a full-time position offering benefits and a wage band of $21.88–$29.65 per hour, BOE.

Qualifications

• Experience Preferred: One or more years of providing Targeted Case Management services and working with children.
• Must have a master's or graduate degree in behavioral sciences or related fields.
• Minimum education includes 30 semester hours or 45 quarter hours in behavioral sciences or related fields.

Responsibilities

• Provide individual, group, and family skills training at school sites, homes, or community.
• Provide targeted case management, crisis intervention, and treatment plan development/review.
• Complete clinical documentation and ensure DHS standards are met in the RMHC EMR.
