Job summary

Sentara Health is seeking an Integrated Case Manager, RN to coordinate comprehensive care for members across the care continuum. The role involves telephonic and in-person assessments, care plan development, and collaboration with medical directors and care teams.

Candidates must have an RN license and at least 3 years of nursing experience; discharge planning and managed care experience are preferred. Remote position requiring residency in specified localities with travel up to 1 hour and

Qualifications

• Associates degree is required.
• Bachelor's degree preferred.
• Registered Nurse licensure required.

Responsibilities

• Provide case management within licensure scope; develop and monitor care plans.
• Conduct telephonic or face-to-face assessments to coordinate member needs.
• Identify high-risk members and coordinate care with the health care team; manage chronic conditions.
• Assist with authorizations/referrals and ensure compliance with policies.
