Job summary

BlueCross BlueShield of South Carolina is seeking an RN Concierge Care Manager to provide high-touch care management to members nationwide, including high-risk populations. The role is fully remote with a standard 40-hour week and some after-hours coverage from home.

You will coordinate treatment plans, navigate prior authorizations, and partner with clinicians, social workers, and case managers to close care gaps. Active RN licensure required, national CM certification within 2 years.

Qualifications

• Associate’s degree in nursing or higher is required.
• RN licensure active and unrestricted in the U.S.
• Case management experience as an RN is required (5 years) or 2 years in a health insurance setting.

Responsibilities

• Directs day-to-day, high-touch member management and care plan coordination.
• Coordinates clinical services with providers, vendors, and social workers.
• Performs team training and supports quality assurance efforts.
• Prepares for external audits and maintains program documentation.
