Job summary

Highmark Inc. is seeking a Community-based Case Manager for the New Castle County area. The RN will perform comprehensive assessments, develop and monitor care plans, and coordinate services across care settings to ensure high-quality, cost-effective care.

The role requires travel within the territory, strong advocacy for members, and adherence to HIPAA and company policies. Remote/work-from-home options are available with regular site visits.

Qualifications

• Bachelor's degree in Social Work or health-related field and 3 years of related experience OR Master’s degree with 1 year of related experience.
• RN/LPN licensure or multi-state licensure (eNLC) with 2 years of related experience in long-term care, home health, hospice, public health, or assisted living.

Responsibilities

• Comprehensive member assessment through in-person visits and ongoing phone support.
• Develop, implement, and monitor individualized care plans addressing physical, behavioral, custodial, social, and psychosocial needs.
• Coordinate access to services, community resources, and specialists across care settings.
• Advocate for members and guide them through healthcare decisions.
• Ensure seamless care transitions between hospitals, community settings, and facilities.
• Monitor progress, evaluate services, and adjust care plans for optimal outcomes.
• Identify opportunities for cost savings, quality improvement, and workflow efficiencies.
