Job summary

Insight Global is seeking Medical Coders specializing in Risk Adjustment Coding. You will review medical records to assign ICD-10-CM codes supporting risk adjustment and HCC reporting.

Ideal candidates have strong medical record review and coding compliance experience, and familiarity with CMS guidelines. Work with providers and clinical teams to improve documentation accuracy and coding quality.

Qualifications

• Experience reviewing physician documentation and assigning ICD-10-CM codes.
• Ability to identify diagnoses impacting risk adjustment and CMS reporting.
• Strong knowledge of CMS guidelines and Medicare Advantage requirements.

Responsibilities

• Review medical records and clinical data to identify reportable diagnoses.
• Assign accurate ICD-10-CM codes based on documentation.
• Validate codes against clinical evidence and standards.
• Conduct prospective and retrospective chart reviews.
• Participate in coding audits and quality assurance initiatives.
• Communicate questions with providers and staff.
• Stay updated on ICD-10, HCC, and CMS changes.
