Job summary

Clarest Health seeks a PV1 Pharmacist to join its Clinical Innovation Center. You will perform the first review of prescriptions to ensure safety and accuracy before fulfillment in a remote or hybrid setting.

You will work with pharmacists, technicians, and prescribers to resolve issues, document interventions, and maintain HIPAA compliance while upholding high clinical standards. Monday–Friday shift with rotating weekends and competitive pay.

Qualifications

• Active Pharmacist license in good standing in the state of practice.
• Minimum 1 year healthcare experience as licensed pharmacist; LTC/Medicare Part D experience preferred.
• Strong clinical knowledge and medication therapy review skills.
• Excellent verbal and written communication skills.

Responsibilities

• Perform PV1 first verification of prescription orders before production/fulfillment.
• Conduct comprehensive clinical reviews for appropriateness based on patient data and allergies.
• Verify information entered by technicians or electronic prescribing systems.
• Identify and resolve discrepancies and incomplete orders.
• Communicate with prescribers to clarify orders, dosing, and documentation requirements.
• Ensure compliance with federal/state regulations and company policies.
• Document interventions and order resolutions accurately.
• Collaborate with pharmacists, technicians, nurses, and facility staff.
• Support quality assurance and continuous process improvement initiatives.
• Maintain HIPAA compliance and confidentiality.
• Complete other duties as assigned.
