Job summary

FamilyWell Health seeks a Perinatal Behavioral Health Coach to join its mission of improving access to compassionate mental health care for pregnant, postpartum, and perimenopausal individuals across the U.S. This remote, part-time role supports patients within the collaborative care model, delivering virtual coaching sessions and coordinating with the care team.

Qualified candidates have at least 1 year of coaching or perinatal/perimenopause experience and will complete the FamilyWell Perinatal

Qualifications

• At least 1 year coaching or perinatal/perimenopause care experience.
• Complete the FamilyWell Perinatal Behavioral Health Coaching Certification within 3 months of start date.
• Passionate about improving perinatal mental health, especially for vulnerable populations.

Responsibilities

• Support the mental health of pregnant and postpartum patients via telehealth for an assigned patient caseload.
• Provide patient education about perinatal mental health disorders and the available treatment options.
• Document patient progress in a HIPAA-compliant EMR and FamilyWell client registry.
• Communicate patient progress with FamilyWell Care Team members.
• Identify patients who are not improving and may need more intensive mental health care and report concerns to the Care Manager and the Psychiatric Consultant.
• Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are nearing discharge.
