Job summary

Qualifications

• Associate’s degree in nursing (ASN) and/or Bachelor of Science in Nursing (BSN) from an accredited school of nursing is required.
• Must have a current unrestricted Registered Nurse (RN) license.
• Minimum of three years of clinical nursing experience.
• Preferred experience in chronic disease management, population health, case management, ambulatory care, home health, telehealth, care coordination, and remote patient monitoring programs.

Responsibilities

• Monitor remotely transmitted patient data and assess trends.
• Collaborate with providers to adjust care plans as needed.
• Educate patients and families on disease management and device use.
• Perform nursing assessments via telehealth or approved platforms.
• Document actions, outcomes, and escalation steps in the medical record.
