Qualifications

• Must hold a Licensed Marriage and Family Therapist (LMFT) license
• Must hold a Licensed Independent Clinical Social Worker (LICSW) license
• Must hold a Licensed Mental Health Counselor (LMHC) license
• Valid NPI number
• Must be able to provide telehealth services
