Cardinal Health is seeking a Nurse, Individualized Care Manager to coordinate medical treatments, deliver patient education, and bridge gaps between patients, healthcare providers and support systems to promote continuity of care.

The role is remote, full-time, and requires California RN licensure and oncology care experience. You will manage a caseload of 100-150 patients, collaborate with onsite and remote teams, and contribute to improved health outcomes through structured care management.

Qualifications

• Registered Nurse with a current, unrestricted California license.
• 5 years’ hospital or clinical experience in complex chronic disease states preferred.
• Care Management experience strongly preferred.
• Oncology patient experience strongly preferred.
• Experience with Microsoft Office products.
• Superior communication skills to include verbal and written.
• Must be able to work collaboratively; team focused.
• Excellent organizational skills.
• Bilingual skills a plus.

Responsibilities

• Collaborate with healthcare staff to develop, implement, monitor and evaluate clinical care and coordinate related activities.
• Manage a caseload size ranging between 100-150 members.
• Coordinate with the oncology care team and care management to assess and modify patient care as needed.
• Document all encounters in the designated EHR system accurately and timely.
• Attend interdisciplinary conferences and team huddles to ensure coordinated care.
• Provide patient education and ensure adherence to care plans.
