Job summary

Eterna Primary Care is seeking a part-time Call Center Nurse/Agent to serve as the first point of contact for patients, families, SNFs, ALFs, hospitals, and pharmacies. The role involves handling high-volume calls, triage, documentation, and coordinating with medical staff.

This hybrid position combines onsite work at the support center with remote options. You will support care coordination, schedule follow-ups, and assist providers with referrals, medication requests, and hospital transitions.

Qualifications

  • Associate degree or higher in nursing.
  • Current and active Registered Nurse (RN) license with multi-state eligibility preferred.
  • Current and active multi-state Licensed Practical Nurse (LPN) license.
  • Experience in skilled nursing or post-acute care preferred.
  • Familiarity with HIPAA guidelines and medical terminology.

Responsibilities

  • Answer incoming calls in a professional, courteous, and compassionate manner.
  • Place outbound calls to patients, family members, facilities, pharmacies, hospitals, and community partners.
  • Verify caller identity following HIPAA guidelines.
  • Document all telephone encounters accurately in the EMR or designated platform.
  • Coordinate with medical staff to triage requests and ensure timely resolution of patient care needs.
  • Escalate urgent clinical concerns immediately.
  • Assist with appointments reminders and follow-up scheduling.
