Job summary

Pharmacy Careers is seeking an MTM Pharmacist to join our growing team. The role focuses on conducting comprehensive medication reviews, coaching patients on adherence, and improving outcomes, often from home or with flexible hybrid schedules.

You’ll collaborate with prescribers, identify therapy issues, educate patients, and maintain CMS MTM program compliance. Ambulatory, hospital, or community MTM experience is a plus, and both new graduates and licensed pharmacists are invited to apply.

Qualifications

• Requires PharmD or Bachelor of Pharmacy degree.
• Ambulatory, hospital, or community MTM experience is a plus; new graduates are encouraged to apply.
• Strong patient counseling, independence, and precise documentation skills.

Responsibilities

• Perform CMRs and targeted med reviews with patients.
• Identify medication problems, interactions, adherence barriers, and cost-saving opportunities.
• Collaborate with prescribers and care teams to resolve therapy issues and document interventions.
• Educate patients and caregivers on safe, effective medication use.
• Ensure CMS MTM program compliance and quality metrics.
