Qualifications

• PharmD or Bachelor of Pharmacy required.
• Ambulatory, hospital, or community MTM experience a plus.
• Licensed pharmacist; new graduates encouraged to apply.

Responsibilities

• Perform comprehensive medication reviews (CMRs) and targeted reviews with patients.
• Identify medication-related problems, interactions, adherence barriers, and cost-saving opportunities.
• Collaborate with prescribers to resolve therapy issues and document interventions.
• Educate patients and caregivers on safe, effective medication use.
• Maintain CMS MTM program compliance and quality metrics.
