Qualifications

• PharmD or Bachelor of Pharmacy requirements.
• Licensed pharmacist status required or new graduates encouraged to apply.
• Excellent patient counseling and clear documentation.

Responsibilities

• Perform Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews with patients.
• Identify medication-related problems, interactions and adherence barriers.
• Collaborate with prescribers and care teams to resolve therapy issues and document interventions.
• Educate patients and caregivers on safe, effective medication use.
• Maintain CMS MTM program compliance and quality metrics.
