Job summary

University Home Care in Warren, MI seeks a Mental Health Therapist to provide social work services in a medical setting. This remote option supports teletherapy with in-person work around Livonia, MI for licensed Michigan clinicians.

Responsibilities include assessments, care planning, collaboration with healthcare teams, and advocacy. A Master’s in Social Work or related field is required, with licensure in Michigan preferred.

Qualifications

• Master's degree in Social Work or related field.
• Therapy license (Preferred).
• Experience as Medical social worker (Preferred).
• Knowledge of medical coding, research, and documentation practices.
• Licensed in Michigan.
• Master's or Doctorate in Psychology, Counseling, or related field.
• Strong interpersonal and communication skills.
• Commitment to providing high-quality care.

Responsibilities

• Provide social work services to patients and their families in a medical setting
• Conduct assessments to determine the psycho-social needs of patients
• Develop and implement care plans based on patient needs and goals
• Collaborate with healthcare professionals to coordinate and facilitate patient care
• Assist patients and families in accessing community resources and support services
• Provide counseling and emotional support to patients and families
• Advocate for patients' rights and ensure they receive appropriate care
• Document patient interactions and maintain accurate records
• Supervise and document limited license medical social workers during counseling sessions
• Review and meet with LMSW to discuss their progress
