Job summary

Xpress Wellness in Stillwater, OK is seeking a Licensed Mental Health Provider (LSCSW or LCPC) for a full-time position in a nursing home setting. You will deliver counseling, conduct assessments, and coordinate care within an integrated primary care behavioral health model.

The role requires a Master’s degree in a mental health field, an active license, and at least 2 years of supervised experience. You will document notes, collaborate with families and healthcare teams, and support patients

Qualifications

Must hold a Master's degree in an accredited mental health field.
Active and unrestricted licensure in state practices (LCSW/LSCSW, LPC, LMFT).
Minimum of 2 years supervised experience in mental health or health care settings.

Responsibilities

Provides mental health counseling, diagnostic evaluation, and assessments.
Produces encounter notes and clinical documentation such as intake assessments and treatment plans.
collaborates with patients, families, and healthcare providers for integrated care.
Coordinates care with facility personnel and referrals as needed.
Maintains productivity expectations and performs other duties as assigned.
