Responsibilities

• Obtain, verify, and document accurate medication histories for hospitalized patients.
• Collaborate with patients, caregivers, pharmacies and healthcare providers to support safe medication management.
• Assist the pharmacy team in ensuring continuity of care across transitions within the healthcare system.
