Job summary

Adaptive Home Health is seeking a Home Health Quality Assurance LPN/LVN (QA LPN) to perform weekend reviews remotely. You will validate SOC/ROC/Recert/Transfer/Discharge documentation, ensure accuracy, and provide coaching to clinicians to improve quality and reimbursement outcomes.

Ideal candidates have an active LPN/LVN license, 2+ years home health experience, and strong knowledge of PDGM. Remote work with weekend schedule offers flexible hours and meaningful impact on patient care and agency

Qualifications

Active, unrestricted LPN or LVN license.
Minimum 2 years of home health experience.
Strong understanding of Medicare home health reimbursement and PDGM.
High attention to detail and strong clinical judgment.
Ability to work independently in a remote environment with minimal supervision.
Excellent written and verbal communication skills.
Proficiency with EMR systems (HCHB preferred) and Microsoft Office.

Responsibilities

Review and validate all assigned assessments (SOC, ROC, Recert, Transfer, Discharge) for accuracy and consistency.
Identify and correct errors impacting reimbursement, quality measures, or compliance before submission.
Provide clear, actionable feedback to field clinicians to facilitate revisions and education.
Support agency compliance with CoPs and internal quality standards.
Identify recurring documentation trends and contribute to clinician training and workflow improvements.
Collaborate with clinical leadership to improve outcomes, reduce audit risk, and maintain defensibility.
Complete reviews within defined turnaround times with high accuracy.
Communicate discrepancies and process improvements promptly with clinicians and leadership.
