Job summary

UnitedHealth Group is hiring a Field Care Coordinator for Blount County, TN to apply clinical knowledge in coordinating member care across acute, home, chronic and community settings, with remote work options for nearby residents.

You will perform assessments, develop and monitor care plans, and educate members and providers. Travel up to 75% within the assigned territory is required, with collaboration across Case Management and Behavioral Health teams.

Qualifications

• Bachelor's degree in Social Work, Nursing or other health care profession including LPN.
• 2+ years of clinical case management experience.
• 1+ years of MS Office experience (Word, Excel, Outlook).

Responsibilities

• Apply clinical knowledge to coordinate member care for medically complex individuals.
• Perform telephonic and face-to-face assessments per policy and timeframes.
• Develop, implement and monitor member-centered care plans.
• Collaborate with Case Management, Utilization Review and Behavioral Health teams.
• Educate members and providers; facilitate access to services.
• Maintain compliant, time-sensitive documentation.
