Qualifications

• Experience developing exercise prescriptions for disease management or special populations.
• Ability to educate, assess, and support behavior change.

Responsibilities

• Collaborates with nurse case managers in managing disease management health goals of members.
• Implements the primary care physicians exercise recommendations providing safe and effective exercise instruction and guidance.
• Provides member health coaching, education, assessment and support of behavioral changes to promote self‑management of chronic conditions.
• Provides members with education materials related specifically to member score condition.
