Job summary

Kaiser Foundation Health Plan of Washington is seeking a Care Manager Liaison Nurse for outpatient complex conditions at Capitol Hill, Seattle, WA. Qualifications

• Minimum three (3) years of RN medical/surgical/ambulatory clinical experience.
• Minimum two (2) years of RN experience in ambulatory case management or care coordination.
• BSN or higher preferred.

Responsibilities

• Assess referred patients, determine needs, and develop patient-centered plans with_timelines.
• Coordinate interventions with PCP/ specialty teams to optimize outcomes.
• Facilitate referrals for home health, long-term care, hospice, or other services.
• Document interventions and communicate effectively with care teams and families.
