Job summary

CommonSpirit Health is seeking a Tele-Pharmacist to work remotely. You will review prescriptions for safety, monitor for drug interactions, and authorize dispensing under supervision to ensure high‑quality care for patients.

You will utilize pharmacokinetic skills to adjust dosing, stay current with practice standards, and communicate effectively with physicians and nurses to maintain continuity of medication therapy across care settings.

Qualifications

• Requires a B.S. in Pharmacy or equivalent from an accredited college of pharmacy.
• Three years of hospital pharmacy experience as a pharmacist.
• Active pharmacist licenses in TX, NE, and CO in good standing.

Responsibilities

• Review physician orders in the patient profile for safety, drug interactions, dosing, allergies, and regimen accuracy.
• Perform pharmacokinetic dosing determinations and monitor patient responses under orders or protocols.
• Participate in continuing education to meet regulatory and organizational goals.
• Support client hospital goals for order entry, verification, and clinical programs.
• Respond promptly to communications from Telepharmacy leadership and client liaisons.
• Provide clear, concise reports during order reviews and monitor for potential patient concerns.
