Job summary

PharmD Live is seeking a licensed New Jersey Clinical Pharmacist to join a virtual care team. This role delivers pharmacist-led clinical services remotely to manage chronic conditions, improve adherence, and achieve positive health outcomes.

You will work with providers and care teams to deliver evidence-based, patient-centered care through telehealth programs while ensuring regulatory compliance and high-quality documentation.

Qualifications

• Active New Jersey Pharmacist License in good standing.
• PharmD degree from an accredited institution.
• Minimum of two years of pharmacist experience in ambulatory care, clinical pharmacy, managed care, telehealth, or related settings.
• Ability to educate patients in Spanish language.

Responsibilities

• Conduct comprehensive medication reviews and reconciliation services.
• Identify, assess and resolve medication-related problems.
• Provide telephonic and virtual patient consultations.
• Support patients with chronic conditions including diabetes, hypertension, heart failure, COPD and other complex disease states.
• Develop individualized medication management recommendations for providers and patients.
• Promote medication adherence through patient coaching and motivational interviewing.
• Document clinical encounters accurately and in accordance with standards.
• Collaborate with physicians, nurse practitioners, and care teams to optimize patient outcomes.
• Contribute to quality improvement and patient engagement initiatives.

Remote Clinical Pharmacist – New Jersey Work Location: Remote (Flexible Remote Schedule) Target Location: New Jersey Residents Preferred License Required: Active New Jersey Pharmacist License (Required)

About the Opportunity

PharmD Live is seeking an experienced Clinical Pharmacist licensed in New Jersey to join our growing virtual care team. This role focuses on delivering pharmacist-led clinical services that help patients better manage chronic conditions, improve medication adherence, and achieve positive health outcomes. The successful candidate will work remotely while collaborating with providers, care coordinators, and interdisciplinary teams to deliver evidence-based, patient-centered care through innovative telehealth programs.

Primary Responsibilities

• Conduct comprehensive medication reviews and medication reconciliation services
• Identify, assess, and resolve medication-related problems
• Provide telephonic and virtual patient consultations
• Support patients with chronic conditions including diabetes, hypertension, heart failure, COPD, and other complex disease states
• Develop individualized medication management recommendations for providers and patients
• Promote medication adherence through patient coaching and motivational interviewing
• Participate in care transition and post-discharge medication review activitiesDocument clinical encounters accurately and in accordance with regulatory and organizational standards
• Collaborate with physicians, nurse practitioners, and care teams to optimize patient outcomes
• Contribute to quality improvement initiatives and patient engagement efforts

Minimum Qualifications

• Doctor of Pharmacy (PharmD) degree from an accredited institution
• Active New Jersey Pharmacist License in good standing
• Minimum of two (2) years of pharmacist experience in ambulatory care, clinical pharmacy, managed care, telehealth, or related settings
• Strong computer proficiency and ability to navigate multiple clinical systems
• Ability to work independently in a remote environment while maintaining productivity and quality standards
• Willingness to obtain additional pharmacist licenses as business needs evolve
• Ability to educate patients in Spanish language
