Job summary

Upperline Health is seeking a Clinical Registered Dietitian to provide nutritional services to a high-risk patient population as part of an interdisciplinary team. Focus areas include weight management, CKD, diabetes, heart health, gout, and kidney stones, with emphasis on compassionate, detail-oriented care.

You will educate the team, assess nutrition status, and develop comprehensive care plans while collaborating in a fast-growing, value-based care environment.

Qualifications

• Registered Dietitian in Florida required; Georgia licensure a plus.
• Experience in adult clinical dietetics preferred.
• Experience working with chronically ill patients and complex cases.
• Comfortable with writing, documentation, and interprofessional collaboration.

Responsibilities

• Educate the interdisciplinary team on dietary guidelines for older adults with chronic illnesses.
• Obtain and evaluate nutrition histories and provide detailed education to patients and families.
• Complete written nutritional assessments and care plans per licensure guidelines; participate in care plan meetings.
• Monitor adherence to nutrition therapy and document patient responses in progress notes.
• Perform related duties as needed by Upperline Health.
