Job summary

Lyrahealth is looking for a Care Navigator to provide crisis management and client support. This fully remote role requires a Master's degree in clinical social work or equivalent, and at least 2 years of relevant experience. You will independently manage a caseload of complex clients and connect them to the appropriate mental health services.

Bilingual candidates are preferred. The position offers flexibility with a shift schedule that includes weekends, alongside a shift pay differential!

Qualifications

• Master's degree in clinical social work or equivalent required.
• Minimum of 2 years of psychiatric intake or related experience.
• Significant experience with high-risk clinical situations.

Responsibilities

• Manage client crises using evidence-based interventions.
• Identify roadblocks to clinical care and connect clients to treatment.
• Independently manage a caseload of complex clients.
