Job description

Piper Companies is currently seeking a Case Manager (RN) for a fully remote opportunity for a highly rated quality health insurance provider.The Case Manger (RN) will be serving as an essential bridge between health care and medical coverage to advocate and increase patient's quality of care, and overall quality of life. This candidate will need an active North Carolina or Compact State RN license.

Responsibilities of the Case Manager:

  • Work on a collaborative Health Care team to monitor utilization patterns to identify and facilitate appropriate health care delivery for selected members
  • Perform comprehensive assessments of member's health status through medical records, support needs, clinical history, and cultural needs.
  • Perform ongoing attention to member's needs including follow-up appointments, and modifications to assess progress
  • Collaborate with primary care physician, medical care coordinator, discharge services, medical director, the member, and family members to develop and coordinate plans of treatment and the proper discharge plans

Qualifications of the Case Manager:

  • RN or LPN- North Carolina OR Compact State License required
  • Prior Telephonic Case Management experience is a plus
  • 3 + years of inpatient care or case management experience
  • Critical care experience, including med-surg, ICU, and etc.

Compensation for the Case Manager, RN:

  • Salary:$32-$37/hr based on relative experience (weekly pay)
  • Comprehensive Benefits including Medical, Dental, Vision, 401k, PTO, Sick Leave if required by law, & Holidays
