Job summary

Community Health Plan of Washington is seeking a Case Manager I to deliver telephonic and field-based case management for members with acute, chronic, and complex needs. You will collaborate with clinicians, caregivers, and community resources to improve quality of life and achieve cost-effective outcomes.

The role requires WA licensure eligibility (RN, SW, LMHC/LMHP/LMFT) and at least 1 year of related experience.

Qualifications

  • Bachelor’s degree in nursing or Master’s degree in social work preferred.
  • Current, unrestricted license in WA as RN, or WA Social Worker license (LSWAA/LSWAIC) or WA LMHC/LMHP/LMFT as applicable.
  • Minimum 1 year case management, home health, discharge planning, or equivalent experience.
  • Minimum 3 years of clinical experience in acute care or outpatient setting.
  • Experience with Microsoft Office products.
  • Case Management Certification preferred.

Responsibilities

  • Perform telephonic case management for members with acute, chronic, and complex needs.
  • Coordinate with providers, members, caregivers, vendors, and community resources.
  • Assess, plan, implement, document, and revise care plans.
  • Initiate and manage care plans with patient/family input.
  • Collaborate with multidisciplinary team to meet goals and access services.
  • Document all case activity in CHPW care management system.
  • Attend interdisciplinary meetings and coordinate with TOC when needed.
  • Travel to member homes, offices, hospitals, or community settings as required.
