Pharmacy Careers seeks a Remote Case Management Pharmacist to coordinate care for high-risk patients from a home office. You will conduct medication reviews, manage complex regimens, and support adherence while collaborating with physicians, nurses, and care coordinators.

Ideal candidates hold a PharmD or Bachelor of Pharmacy, have MTM or managed-care experience, and excel at communication, problem-solving, and care coordination.

Qualifications

• Education: PharmD or Bachelor of Pharmacy.
• Experience: Case management, MTM, or managed care preferred; hospital, ambulatory, and community pharmacists with strong counseling backgrounds encouraged.
• Skills: Strong communication, problem-solving, and care-coordination abilities.

Responsibilities

• Collaborate with physicians, nurses, and care coordinators to manage high-risk or complex patients.
• Conduct medication reviews to identify gaps in therapy, adherence concerns, or potential interactions.
• Support prior-authorization and appeals processes when needed.
• Educate patients and caregivers on regimens and disease-state management.
• Document case activities and outcomes in compliance with health-plan and regulatory standards.
• Participate in quality initiatives aimed at reducing hospitalizations and improving outcomes.
