Job summary

Net Health is seeking a Care Navigator (PTA) to support patients remotely, guiding home-exercise programs and therapeutic activities through our platform. You will monitor progress, engage users, and coordinate with clinicians to achieve better musculoskeletal health outcomes.

The ideal candidate has PTA licensure in New York, a accredited PT program background, and a collaborative, tech-savvy approach to deliver patient-centered care in a fast-growing, work-from-anywhere startup environment.

Qualifications

• PTA with NY license and graduation from an accredited physical therapy program.
• 2+ years in musculoskeletal health is a plus.
• Experience in behavioral modification is a plus.
• Excellent communication and data presentation skills.
• Ability to work independently and cross-functionally.

Responsibilities

• Provide remote monitoring services to support patients in the home-exercise program.
• Direct and assist users in therapeutic exercise, neuromuscular re-education, and functional training via the platform.
• Provide ongoing feedback to clinicians on patient progress outside of the clinic.
• Maintain records and notes while monitoring patient progress.
• Assist in improving processes and functionality of the platform.
• Organize and manage a caseload efficiently.
